The °®°®Ö±²¥ cares about the experiences of our students, faculty, staff, and community. To ensure we are promoting a responsive, caring, safe, inclusive, and ethical environment, we need to know about your experiences. The links below facilitate the reporting of concerning behaviors, student concerns and violations of our policies and procedures, as well as provide access to other resources to help address these concerns.

These reports are not monitored 24/7. If you believe that a situation you are reporting is an emergency and requires immediate attention, please call 911 to contact first responders.

Students, Faculty, Staff and Community

Refer Student for Concerning Behavior or Needed Support

Utilize this form if you have personal concerns about a student (or yourself) which would impact their success or present a danger or threat to themselves or the community. With as much detail as possible, describe what behavior, statements, observations, etc., have prompted this referral and, when applicable, provide specific examples. This information will be shared with on-campus professionals who will contact the referred individual to assist. 

Report Student Conduct Violations

Utilize this form to report an alleged non-academic violoation of the Student Code of Conduct including but not limited to, alcohol, disorderly conduct, drugs, hazing, harassment, physical violence, or other violations of university policies, regulations, or rules and/or conduct that violates state or federal laws.


Report a Title IX Incident

Utilize this form to report incidents of sex discrimination, including discrimination based on gender, sexual harassment, sexual violence, relationship/domestic violence, stalking, and/or discrimination based on pregnancy. 

Report Incidents of Misconduct, Fraud, or Abuse

°®°®Ö±²¥ has engaged EthicsPoint, a third party hosted hotline, to enable the safe, secure, and anonymous reporting of activities which may involve misconduct, fraud, abuse, and other violations of °®°®Ö±²¥ policies. Report Equal Opportunity (EO) Violation

Utilize this form to report incidents of protected class discrimination, harassment, or retaliation based on protected class reporting.

Request Disciplinary Certification or Records

Utilize this form if you need a disciplinary certification for another school or a disciplinary records release for other purposes.

Students Only

Report an Academic Concern

If you have unsuccessfully attempted to resolve the issue directly with your instructor and, subsequently, their department chair, then utilize this form to contact the Dean of Students.

Submit a Formal Academic Grievance 

Students may file a formal grievance for a course that a specific academic decision or action has violated published polices and procedures. Disagreement with individual test or assignment grades will not be considered grounds for an academic grievance unless they impact the final grade. Students should reference for the procedures to file this grievance.


Report a Non-Academic Concern 

Utilize this form if you (or a °®°®Ö±²¥ student you know) are: (1) experiencing personal or medical concerns that are negatively impacting you and you need assistance navigating your °®°®Ö±²¥ options, (2) having an issue with a °®°®Ö±²¥ office after first trying to resolve it with that office, (3) need assistance with finding the appropriate office to address your concern.

Submit a Formal Non-Academic Grievance

If you are not able to resolve your concerns with the person most directly involved with your complaint through informal channels first, utilize this form to formally  report alleged improper, unfair, or arbitrary action by a °®°®Ö±²¥ department, administrator, and/or staff member involving the application of a specific provision of a °®°®Ö±²¥ regulation, policy, or procedure.

Utilize this form if you are experiencing academic or institutional barriers affecting your academic progress or ability to continue at °®°®Ö±²¥. You will be contacted by an Academic Advocate who will assist in you with options and resources.

Faculty and Staff Only

Report an Academic Disruption

The provides the steps an instructor may take to immediately address student disrupting a class or academic setting including restricting a student from class. Instructors may utilize this form to make a referral to Student Conduct & Ethical Development (SCED) for a comprehensive review and additional sanctions separate from the academic process.

Faculty can utilize this form or the link in their Canvas class to report any issue of academic dishonesty involving a graduate student, which includes behaviors such as cheating, plagiarism, fabrication, forgery, misuse of technology or intellectual property, or other violations as defined in . All reports are reviewed by the course’s academic department and tracked by °®°®Ö±²¥ Academic Affairs and Student Success units. Refer to the regulation for process information. 


Report Undergraduate Student Academic Integrity Violation
